childkind_home_based_imageWhen pediatricians or medical specialists have a concern over the ability of a parent to manage their child’s complex regimen of care, they can call Childkind. Working in partnership with the medical staff and the family members themselves, Childkind staff will develop and implement a Family Care Plan and, if needed, a Medical Teaching Plan.

A Continuum of Care

Childkind’s Home Based Services program is organized along a continuum, relative to a family’s ability to comply with their child’s prescribed regimen of care. Children at the highest risk for secondary health complications, frequent hospital readmissions, or foster placement receive our intensive Supportive Care Coordination services.

Least Intensive

Advocacy & Training (A&T)

A&T is provided to families needing assistance navigating the healthcare of support service system but do not require a home visit. A&T is provided over the phone, via e-mail, or within group trainings.

Intensive

Family Support Services (FSS)

FSS is offered by a Masters Level Social Worker to families when social or community circumstances impede compliance to a child’s health care requirements or when a child’s presenting diagnosis is developmental in nature. FSS is provided in the family’s home.

Most Intensive

Supportive Care Coordination

SCC is designed for families of children with complex health care needs. It is provided by a Registered Nurse and Licensed Masters Level Social Worker team when medical compliance is a significant challenge. SCC is provided in the family’s home.
